Cars going straight on Arch street in the left lane collide with cars in right lane making a left on 4th from Arch Street.
Sending the turning car in to the Holiday Inn wall. Pedestrians have already been critically injured because of this.

This intersection experiences worst outcome, because cars usually speed to go through the yellow light.

As simple fix: as cars approach the 4th street intersection: bolder lane lines.
Lastly an arrow (NO TURNS) is needed in the right lane (in front of the firehouse).
