This has probably been reported 10+ times in 2 years on here. SeeClickFix can't help because it's DOT responsibility and DOT won't participate with SeeClickFix. I've reported it and followed up multiple times through the DOT's protocol with little to no response (occasional canned responses).

Here's my copy/ paste of one of my report threads on this issue to the DOT (so you have my confirmation # if you want to reference):
I keep all my tracking numbers. I started reporting in February 2015. Here's a whole thread I've kept ongoing since July 2015.

Heading west on Millbrook toward Sertoma Arts Center, there are two extremely large potholes. The one in the right lane is deep enough to damage a car and there is also an area on half of it that is all built up with asphalt in a strange way. Just ahead of that in the left lane, there is a large but slightly less deep pothole. This is about two blocks before the entrance to Sertoma Arts Center.

The Road Maintenance Supervisor will review your concern, prioritize and schedule any necessary repair work.

This pot hole is worsening and has not been addressed. The road construction is obviously failing in that section on both sides of the road as there is noticeable damage to the concrete that is not just a pot hole. Did the road maintenance supervisor view the area?

These are still there and worsening. This has been going on more than a year. Unacceptable - someone is going to get hurt. I've saved all my reports, but I'm not sure how to stop being ignored by the DOT. I'm following your protocol.

Thanks for reminding me that it is an NC DOT issue. Just reported it here:

The tracking number is MQY2QIAP2Y
The problem is that the road was laid in clay with insufficient drainage. When we get heavy rains, water "boils" up under the asphalt and lifts the surface paving off.
