Description
Third Request....Repairs were made to the light pole in the alley and the temporary patchwork around that repair is sinking. Permanent seating around the repair before someone hurts themselves.

Comment: THE ON SITE INSPECTION REVEALED THAT THE ASPHALT ALLEY HAS A LOW DC ELECTRIC BOX IN THE WALK PATH AT THE BACK DOOR OF SR ADDRESS, ALSO A BELOW EXISTING GRADE MANHOLE AND SEVERAL POTHOLES. THE RESIDENT SAID THAT SOME OF THE POLE LIGHTS WERE NOT COMING ON AT NIGHT. Per Inspector SMITH
The requested service is for Pothole Repair. This has been forwarded to Street and Bridge Maintenance for scheduling.
