Opis

Rubber floor material at the swings has been torn off. A section measuring about 1 1/2 × 1 1/2 & about 4 in. Depth.
My son tripped on this hole. He's fine, but could have easily twisted his ankle or worse. This is a hazard for my kids as well as the other kids running around.
There are other holes near the climbing pad that aren't as big but still a hazard.
