I reported this a few years ago and you had a wonderfully quick response. They ground it down flush. However since then it has heaved up more. Please cut this square of concrete out and re-pour it. That should last longer. Thank you.

It has caused a 1 inch raise in the section making it a trip hazard. Please repair that section of the sidewalk. Probably only one section will need replaced.
