Homeowner requests maintenance of overgrown City of Richmond easement behind 6105 Wesley Drive and 6102 Bay Street. The easement is accessible via the alleyway which runs between Wesley and Bay.

COR DPW employee reports sanitary sewer odor in wooded area adjacent the Commerce Road bridge over Goode's Creek. Odor linked to missing sewer manhole cover on 27" sanitary main. Request replacement manhole cover be installed at this location. Approximate Lat. / Long is: 37.493837,-77.43408

Two blocked storm drain inlets at the intersection of 25th and Main are causing back-up / flooding into the intersection. Requesting inlet maintenance @ COR DPU Stormwater Maintenance.

c/o DPU Stormwater Maintenance, Homeowner on Green Acres Road requests maintenance to ill-defined drainage ditches along Mobile road which cause runoff to backup onto his property. Whether storm water ditches in this area are owned and/or maintained by COR is unclear.

Citizen requests the installation of a "No Littering" / "No Dumping" sign or City-maintained permanent waste receptacle to deter litter and illegal dumping of waste near her property and the nearby waterway.

Citizen flagged down COR employee to report flooding around the drop inlet in front of her house. COR employee inspected the interior of the drop inlet vault and found the discharge pipe approximately 70% blocked with leaf litter. Request maintenance service for this drop inlet.

Resident states that some of the sidewalk in front of 4313 Stuart Avenue was removed in August of 2013 in order to allow a private plumbing company to replace the sewer lateral from the house to the sewer main. Resident is requesting replacement of the removed sidewalk. Resident seemed concerned about pedestrian safety as well as the time frame between the sidewalk's removal and its eventual replacement.

9 used tires were observed to have been dumped along Formex Road. Additionally, 20-30 used tires were observed to have been illegally dumped down the slope adjacent to Broad Rock Creek. Requesting bulk pick-up as soon as practicable.

Miscellaneous furniture and debris was observed to have been illegally dumped along Dale Avenue

Semi truck in disrepair on Formex Rd. Truck has been on site for over 30 days. Individual was seen working on the truck on 6/28/13 around 11:30am on Formex Rd. Parcel address of vehicle location is 3409 Formex Rd but is parked on the back side of the property. Marker on map indicates location.

Citizen has reported a damaged hydrant along Stony Run Road adjacent to the new constructed concrete staircase near the BMX track at Gillies Creek Park. Citizen requests repair/replacement of the hydrant unit. Please find the attached photo.

Mr. James Motley has reported an overgrown lot at 2404 Grayland Ave, and overgrown property along the north side of Grayland Ave from S. Stafford Ave west to S. Robinson St.
Mr. Motley also reports a safety concern regarding trees located at the corner of Grayland Ave and S. Stafford Ave. He says that visibility while driving is badly restricted due to the trees.

The pet waste station in Libby Terrace Park is out of pet waste collection bags and requires restocking.

Request maintenance of partially blocked stormwater conveyance and drainage ditch rehabilitation along Garden Road adjacent to J.B. Fisher Elementary School to ensure stormwater drainage sytem is functioning as designed.

Resident states rain events produce high volume flows down Old Dominion Street. Requesting an investigation of storm inlets along Decatur Street between E. Clopton and W. Pilkington Street to ensure these drains are clear of debris and functioning as designed.

Manhole cover for storm drain inlet located on the west side of Formex across the street from Old Dominion Freight Line is missing.
