This tree has been reported multiple times and no action has been taken. The tree is clearly dead, bark and branches are falling off and it poses a hazard on a very busy circle of Monument Avenue.

The city needs to remove this tree immediately to avoid serious injury to a person or costly property damage.

This lamp post outside of the One Monument Avenue building has been flickering for awhile. Last night during the rain when water infiltrated the fixtures the lamp started sparking internally and smoke could be smelled. Please have someone examine this unit for repair of the issue and replacement of the lamp.

This tree has been dead for over a year and is dropping numerous large branches on cars and pedestrian areas below. Please remove this hazardous tree before the entire decayed trunk comes tumbling down on someone.

It looks like water from a pipe below Monument Avenue is leaking and causing water to accumulate on the northeast corner of Stuart Circle. This accumulating water will eventually be a hazard as overnight temperatures cause this water to freeze into icy patches on this busy thoroughfare.

Theres a couple deep potholes that have formed on Park east of the intersection with Lombardy.

A streetlamp is burned out on Stuart Circle outside of the One Monument Avenue building at 413 Stuart Circle.

Large amounts of litter have filled the storm drain on Ryland at the intersection with Franklin. Please schedule this for debris removal.

A bike is stripped down and abandoned at a street sign at the intersection of Harrison and West Franklin St. Please ticket and remove.

Last month the permanent stop sign at West Ave/Lombardy was destroyed and the city put up a temporary sign. Unfortunately after the rally this weekend city staff inadvertently removed this sign and now this busy intersection has no signage. With so many visitors to the neighborhood this coming week for college move in this sign needs immediate replacement.

A dead tree adjacemt to the UCC needs to be removed before its decaying branches fall and hurt someone.

The stop sign at the intersection of West Avenue and Lombardy is on the ground and the intersection now has no signage. This could become very dangerous very quickly as drivers don't yield to oncoming traffic.

The overhead street lamp on Lombardy at the intersection with West Avenue is no longer working and this block is exceptionally dark at night without that illumination. Please have this repaired.

Lamp is burned out in light on the corner of Monument and Lombardy adjacent to the Lutheran Church.

The spotlights on the Stuart Monument seem to be misaligned and aimed above the actual monument and are not shining on it.

The lamp at Stuart Circle/Lombardy and Monument Avenue is burned out and needs repair.

The lamp on the southern side of W. Franklin Street is burned out and needs replacement.

50% of the canopy of this tree is dead and the tree either needs trimming or replacement to prevent property damage or injury.

A large portion of the top of this tree is dead and needs to be trimmed before it falls and damages property or hurts a person.

Northbound traffic on Lombardy cant see the traffic signals due to foliage growth around traffic lights.
