On the iPhone app, Issues button, I see a list of dozens of issues, many dating from years ago. Also, the "map" function is totally flakey. Zooming out, I see a cluster of pins, but if I move the map by finger drag, the pin cluster remains centered even though the map has shifted.

True, the list of issue shows reports that were made well before Concord joined the program. We have had very few issues reported since launch. Over time the list will reflect current events.
The map function will report any issue through the entire Commonwealth connect system. Dragging the map to another town will show the issues for that town. If you were to click on one of those pins, you'd see that they are not the same issues as the pins in the Concord vicinity. Many communities throughout Massachusetts are using SeeClickFix. And, as a user you can report issues where ever you find them, even in communities that do not have the program set up yet, those issues are always displayed on the map associated with the address or location from which they are reported.

As noted in the previous comment. Maps span communities and can contain information for other municipalities not participating directly at this time. There are also a few old issue from prior to Concord's participation.
I hope I have clarified how the system works. Feel free to provide additional feedback on features.
